Instructions
1. Download the Maintenance Plan Template below. Complete with your practice’s plan. Upload your Maintenance Plan.
2. Continue to use VHealth to guide your practice in maintaining all four of the changes you made during the MOC projects.
3. Review the data feedback reports to track your progress and make adjustments to your processes as needed.
